From the school nurse

Mrs. Laura Levine, R.N. keeps the health records for every child in the building. These are the forms she needs, and when.

The following information will help students have a safe and healthy year at Holy Spirit School. Call the nurse's office or email the school to reach the nurse with any questions.

Paperwork for the upcoming school year

Students must have a current physical less than one year old. A current copy of the immunisation record should be attached, with any additions.

Incoming Sixth grade

It is a requirement in the State of New Jersey that all incoming sixth graders who are 11 years old receive the DTaP and meningococcal vaccines prior to entry into sixth grade. Sixth graders under 11 must receive the vaccine once they turn 11. Submit a copy of these immunisations along with the yearly physical before the first day of school.

Incoming Kindergarten

Submit a copy of the five-year-old physical and additional required vaccines. This usually includes the second MMR and additional doses of both the IPV and DTaP vaccines since Pre-K enrolment. This must be received before the first day of school.

Grades 5–8 sports — cheerleading and basketball

Submit the Annual Athletic Pre-Participation forms by 1 October. Athletic forms must be submitted before the first practice, or your child will not play.

  • Medical Eligibility form, completed, signed and dated by the child's physician. Print and bring all forms to your physician.
  • Health Update form, if the athletic physical is more than 90 days before the first practice.
