What the nurse needs on file, by grade — and when. Forms are available from the nurse or the main office.

All new students

History and physical exam completed within one year prior to admission. Students in Pre-K–4 use the physical form given at registration; students in Grades 5 through 8 use the form provided in the medical forms section.

Incoming Kindergarten

Submit a copy of the five-year-old physical and additional required vaccines. This usually includes the second MMR and additional doses of both the IPV and DTaP vaccines since Pre-K enrolment. Per State law this must be received before the first day of school, or the child cannot be admitted.

Upcoming Sixth grade

Submit a copy of DTaP and meningococcal vaccines. A copy of a physical is not required, but is appreciated.

Grades 5–8 sports — cheerleading and basketball

Submit annual Athletic Pre-Participation Forms Part A and B, completed and signed by both parent and health care provider. The date of the physical exam must be within one year of 1 October of the current school year.

If the physical was performed more than 90 days prior to try-outs, students must also submit a Health Update Form completed and signed by a parent.
